Submitting a HIPAA Agreement Form

1. Navigate to the Internal

2. Go to Compliance and select HIPAA

3. Select the Setting icon and from the action dropdown select Live.


Note - This will take you to the HIPAA Agreement form
Patients can also access this form from your website
Users can send an invite from dashboard to patients 

- Fill out the whole form
Note: All fields in red are "required" and yellow are "optional" to fill up!

4. Get Started

View Agreement Verbiage


Signer Information

Signer Address


Signer Contact

Submitter will receive status notifications via SMS/Email 

Confirmation Page
- Select submit once the form is completed

Submitter must Sign and date and will be sent notification email and sms

Submission will show in HIPAA table as Submitted
